Position Summary:

Care Coordinator is a professional and highly specialized position serving as the point of contact for connecting clients to clinicians for biopsychosocial evaluations. This Coordinator utilizes Great Client Care skills when assigning clients who walk in for a biopsychosocial evaluation to the clinicians. This Coordinator utilizes Mental Health First Aid training skills to triage clients who present critical mental health issues using the client questionnaire form. This Coordinator maintains organization of the Centralized Intake process working closely with Access and Admissions Department leadership and staff to provide quality and professional services to all clients seeking same day walk in biopsychosocial evaluations.

Supports clinical and administrative professionals on the Centralized Intake Team, Access and Admissions leadership and Meridian's mission by ensuring that service coordination work is effective, efficient, and consistent, provide positive client experiences, timely communication, and consumer participation in treatment.

Minimum Requirements:

  • 2 years working directly with community intake processes, working a front-line high-pressure role.
  • This may include experience in working in an office setting, scheduling, working on an electronic medical records (EMR) program, medical scheduling.
  • High school diploma or GED Equivalent


Essential Duties/Responsibilities:

1. Effective and efficient communication with immediate supervisor written and orally 100% of the time.

  1. Ensure all walk-in clients are assessed for immediate needs and triaged appropriately.
  2. Communication of wait time to clients if wait time will be greater than one hour.
  3. Maintains contact with all Meridian offices related to clients seeking biopsychosocial evaluations using the Centralized Intake Channel.
  4. Serves as a liaison between walk in clients and clinical staff.


Other Duties/Responsibilities:

  • Works independently throughout most of the day.
  • Works a 40-hour week and receives prior approval before working more hours.
  • Schedules PTO timely by submitting requests to the Supervisor to ensure the program needs are met for coverage.
  • Performs other duties as required to meet program and departmental goals and objectives.


Position Requirements/Preferences: High School or Associate of Arts Degree in business management, scheduling and medical services office professional.


Drivers Requirement:

Must have a current Florida Driver's License with a clean driving record while meeting company insurance requirements.

In order to qualify as a Center Driver, the candidate must be at least 21 years of age and have an active Florida Driver's license. Motor Vehicle Record will be reviewed for eligibility. No more than 3 moving violations and/or accidents within the past 3 years and no major violations during the past 5 years. Drivers over 65 years old must provide a current doctor's note stating they have no driving limitations.

Knowledge, Skills, Abilities:

Specific competencies required are identified on a department/program specific basis and identified in each employee's Competency Checklist. The following are general and apply to all positions with this title.

Language Skills:

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to draft routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.

Mathematical Skills:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage and to draw and interpret bar graphs.

Reasoning Ability:

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. The ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
