- Completes comprehensive assessments, assessing the total needs of the client and ensuring that there is an opioid use disorder.
- Develops a service plan with specific goals and objectives to resolve service needs with the client's input and therapist agreement (Links the client and family with services and resources identified in the Service Plan, Provides linkages for childcare services and maintains contact with all staff/agencies providing services for the purpose of monitoring clients' progress and assessing appropriateness of services.)
- Serves as an advocate for the client with a wide variety of community agencies and Center programs. Promotes community program development to meet client needs.
- Conducts face-to-face visits to monitor progress.
- Prepares and maintains client records, insures quality of documentation of medical records and records every activity engaged on behalf of a client on a daily basis via Progress Notes
- Performs other duties as required to meet program and departmental goals and objectives.
Who are we looking for? (Minimum Qualifications)
- Bachelor's Degree from an accredited university with major course work in the areas of healthcare, psychology, or social work.
- 1-year experience in Care Management with primary care, SMI, SUD or similar populations required.
- Must have a current Florida Driver's License with a clean driving record while meeting company insurance requirements.
