As a Care Coordinator with Meridian's Court Diversion program, you'll have the opportunity to be an advocate, a resource, and a source of support for individuals navigating both behavioral health needs and the criminal justice system. WHAT YOU'LL DO
As a Care Coordinator, Court Diversion, you'll work closely with clients, families, treatment providers, community agencies, and members of the legal system to coordinate services and help individuals successfully navigate their care.
Your responsibilities may include:
- Complete comprehensive assessments to identify each client's individual strengths, challenges, and service needs.
- Work collaboratively with clients to develop a Plan of Care with meaningful goals and objectives that support their individual needs and recovery.
- Connect clients and their families with treatment, support services, and community resources identified in their Plan of Care.
- Maintain communication with providers, agencies, and other members of the client's support network to monitor progress and ensure services continue to meet their needs.
- Advocate on behalf of clients with community agencies, legal entities, court representatives, and Meridian programs.
- Conduct face-to-face visits to monitor client progress and provide support in a variety of settings, including clients' homes, courtrooms, jails, residential programs, and inpatient psychiatric facilities.
- Collaborate with court personnel, jail staff, public defenders, State Attorney representatives, treatment providers, and other community partners.
- Maintain accurate and timely client records, progress notes, and required documentation.
- Document services and activities provided on behalf of clients in accordance with program and organizational requirements.
- Research, develop, and maintain knowledge of community resources available to support clients and their families.
- Help clients navigate barriers to services while promoting engagement, independence, stability, and recovery.
- Perform additional duties as needed to support the goals of the Court Diversion program and the individuals we serve.
WHAT YOU'LL BRING
Education & Experience
- Bachelor's degree in Social Work, Psychology, or a related field.
- Minimum of 1 year of experience working with mental health populations.
- Forensic or criminal justice experience is preferred.
- Experience working with individuals who may have complex behavioral health, social, or community-based needs is valuable in this role.

- Adaptability: The ability to work effectively in different environments, including community settings, courts, correctional facilities, residential programs, and behavioral healthcare facilities.
- Teamwork: A collaborative approach to supporting clients and working alongside clinical staff, community agencies, and legal partners.
- A commitment to treating every individual with dignity, respect, and compassion.
TRAVEL & COMMUNITY-BASED WORK
This role goes beyond the traditional office setting. Supporting our clients may mean meeting them wherever services and advocacy are needed.
- A valid Florida driver's license and acceptable driving record are required.
- Local travel may be required to support clients and coordinate services.
- Face-to-face client contact may take place in homes, courts, jails, residential programs, inpatient psychiatric facilities, Meridian locations, and other community settings.
- Candidates should be comfortable working collaboratively with behavioral healthcare providers as well as court, correctional, and community partners.
