What You'll Do
- Completes comprehensive assessments, assessing the total needs of the client.
- Develops, with the client's input and agreement, a Plan of Care with specific goals and objectives to resolve service needs.
- Links the client and family with services and resources identified in the plan of care promoting recovery.
- Maintains contact with all staff/agencies providing services for the purpose of monitoring clients' progress and assessing appropriateness of services.
- Serves as an advocate for the client with a wide variety of community agencies, legal entities, and Meridian programs.
- Conducts face-to-face visits to monitor progress in implementation of the plan of care, including home visits, court appearances, jail, residential units and inpatient psychiatric care facilities.
- Prepares and maintains client records and ensures quality of documentation of medical records.
- Records every activity engaged on behalf of a client on a daily basis via Progress Notes.
- Researches, develops and maintains a comprehensive list of community resources that includes addresses, phone numbers, and names of contacts for each resource, including jail personnel, court representative, public defender/ states attorney and assistant.
- Performs other duties as required to meet program and departmental goals and objectives
What We're Looking For
- Bachelor's degree in Social Work, Psychology, or a related field
- Minimum of 1 year of experience working with mental health populations
- Forensic or criminal justice experience (preferred)
- Valid Florida Driver's License with a clean driving record
