Discharge Planner

Gainesville, FL | $19.82/hr. | non-exempt

As a Discharge Planner, you'll collaborate with an interdisciplinary team to coordinate safe, effective transitions from inpatient care. You'll work closely with clients, families, providers, and community partners to ensure continuity of care and support recovery.

• Coordinate discharge planning and follow-up care
• Collaborate with clinical teams to support treatment goals
• Conduct family meetings and gather collateral information
• Participate in treatment team meetings
• Support therapeutic groups and patient engagement

• Bachelor's degree in social work, psychology, counseling, or related field
• New graduates encouraged; experience in discharge planning or case management preferred
