As the TIEH Team Lead at Meridian, you'll lead a multidisciplinary team dedicated to supporting individuals experiencing or at risk of homelessness. Working alongside Care Coordinators, a Clinician, an Outreach Specialist, and a Peer Support Specialist, you'll help your team build meaningful relationships with individuals in the community and connect them with the services and resources they need to move toward greater stability, independence, and recovery.
This is not a role that's limited to the walls of an office. You and your team will be present in the community, including shelters, client homes, areas near homeless encampments, community events, and Meridian locations. WHAT YOU'LL DO
As the TIEH Team Lead, you'll provide hands-on leadership to an interdisciplinary team providing outreach, care coordination, clinical services, and peer support to individuals experiencing or at risk of homelessness. You'll help your team meet individuals where they are, identify their needs, and connect them with resources that support recovery, independence, and long-term stability.
Your responsibilities may include:
- Lead and support an interdisciplinary team of Care Coordinators, a Clinician, an Outreach Specialist, and a Peer Support Specialist.
- Oversee community outreach efforts to identify, engage, and connect individuals who may be eligible for TIEH services.
- Build and maintain strong relationships with community stakeholders, referral sources, homeless shelters, service providers, and other community partners.
- Identify and participate in targeted community outreach events to increase awareness of available services and connect individuals with care.
- Work with clients and members of the care team to develop individualized service plans with meaningful goals, including progress toward stable housing.
- Connect clients with services and resources that support their individual needs, including psychiatric care, substance use treatment, medical and dental care, employment assistance, temporary shelter, and longer-term housing.
- Advocate for clients while working collaboratively with community agencies and Meridian programs.
- Maintain communication with agencies and providers involved in a client's care to monitor progress and evaluate the effectiveness of services.
- Guide the TIEH team in implementing best practices for serving individuals experiencing homelessness, behavioral health challenges, substance use concerns, and other complex needs.
- Conduct face-to-face visits to monitor client progress toward treatment and service plan goals.
- Support outreach and service delivery in community-based settings, including homeless shelters, client homes, areas near homeless encampments, and Meridian clinics.
- Monitor individual and team productivity, impact hours, and applicable revenue goals.
- Research and maintain current information about community resources available to support clients.
- Ensure timely, accurate, and high-quality client records, medical record documentation, and daily progress notes.
- Conduct peer reviews and chart audits to support quality of care, documentation standards, and compliance.
- Support required data collection and data entry associated with the program's federal grant.
- Perform additional duties as necessary to support TIEH program goals and the individuals and communities we serve.
WHAT YOU'LL BRING
Education & Experience
- Master's degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or a related field.
- Experience working with individuals experiencing or at risk of homelessness is preferred.
- Experience serving individuals with mental health and/or substance use challenges is preferred.
- Previous experience in community outreach and care coordination is preferred.
- Previous supervisory, leadership, or management experience is preferred.
- Experience building relationships with community organizations and connecting individuals with services and resources is valuable in this role.

- Quality & Compliance: Strong attention to documentation and the ability to support accurate records, peer reviews, chart audits, and program requirements.
- Problem-Solving: The ability to navigate complex situations, identify resources, and work collaboratively toward practical solutions.
- Adaptability: The ability to work effectively in a variety of community environments and respond to the changing needs of clients and the program.
- Teamwork: A collaborative approach to ensuring clients receive coordinated, compassionate, and meaningful support.
TRAVEL & COMMUNITY-BASED WORK
This role goes beyond a traditional office setting. The TIEH Team works directly within the community to build relationships and connect individuals with services where they are.
- A current Florida driver's license and acceptable driving record are required.
- Candidates must meet Meridian's motor vehicle and company insurance eligibility requirements.
- Center Driver eligibility requires candidates to be at least 21 years of age with an active Florida driver's license.
- Driving eligibility includes no more than three moving violations and/or accidents within the past three years and no major violations within the past five years.
- Regular community-based work may include homeless shelters, client homes, areas near homeless encampments, community events, Meridian clinics, and other locations where individuals can be connected with services.
- Candidates should be comfortable working independently in community settings while maintaining close collaboration with their team and organizational leadership.
