Care Coordinator

$3,000 hiring incentive bonus for specific programs inquire within! 

Salary Range: $19.23/hour

Locations: Gainesville, FL, Chiefland, FL, Lake City, FL, Ocala, FL, MacClenny, FL.

This is an independent professional position performing assessments, service brokerage, and advocacy services for adults with opioid use disorders who are in need of Medication Assisted Treatment (MAT) services. The Care Coordinator coordinates the care for consumers and works with other professionals from numerous community organizations, as well as with Meridian staff. The work is performed with a high degree of independence.

Minimum Qualifications: 

BA/BS from an accredited university with major course work in the areas of health related/ psychology/social work areas with at least 1-year experience in Care Management with primary care, SMI, SUD or similar populations.

Drivers Requirement:

Must have a current Florida Driver's License with a clean driving record while meeting company insurance requirements.
In order to qualify as a Center Driver, the candidate must at least 21 years of age and have an active Florida Driver's license.  Motor Vehicle Record will be reviewed for eligibility.  No more than 3 moving violations and/or accidents within the past 3 years and no major violations during the past 5 years. Drivers over 65 years old must provide a current doctor's note stating they have no driving limitations. 

Essential Duties/Responsibilities:

  • Completes comprehensive assessments, assessing the total needs of the client
  • Ensures that there is an opioid use disorder
  • Provides comprehensive SOAR services so clients can apply and obtain benefits
  • Develops a service plan with specific goals and objectives to resolve service needs with the client's input and therapist agreement
  • Performs drug screens (same sex for urine drug screen collections)
  • Links the client and family with services and resources identified in the Service Plan
  • Provides linkages for childcare services
  • Maintains contact with all staff/agencies providing services for the purpose of monitoring clients' progress and assessing appropriateness of services
  • Serves as an advocate for the client with a wide variety of community agencies and Center programs. Promotes community program development to meet client needs.
  • Conducts face-to-face visits to monitor progress
  • Prepares and maintains client records and insures quality of documentation of medical records
  • Records every activity engaged on behalf of a client on a daily basis via Progress Notes
  • Researches, develops and maintains a comprehensive list of community resources that includes addresses, phone numbers, and names of contacts for each resource
  • Performs other duties as required to meet program and departmental goals and objectives

Understands how abuse and trauma needs to be addressed

in treatment Understands how to effectively integrate family/caregiver in treatment

Understands the nature of and maintains appropriate professional boundaries (e.g., can examine the use of self in therapy, transference/counter-transference issues)

Provides culturally competent care.
