About the Role
As the Crisis Counselor, you will play a pivotal role in shaping the operational excellence and strategic direction of Meridian. You will:
- Table Outreach and Recovery Specialist events. Provide feedback to project manager on presenting needs of the community.
- Assist in providing services as outlined in the community Outreach and Recovery Plan of Action to assist in meeting deliverables of grant.
- Maintains contact with all staff/agencies providing services for the purpose of monitoring clients' progress and assessing appropriateness of services.
- Serves as a resource to citizens and other community agencies.
- Track/tally required services provided and Meet quality and quantity of services benchmarks.
- Records every activity engaged in on behalf of a client on a daily basis in electronic health records. Researches, develops and maintains a comprehensive list of community resources that includes addresses, phone numbers, and names of contacts for each resource.
- Performs other duties as required to meet program and departmental goals and objectives.
What We're Looking For
- High School or Associate of Arts Degree, BA/BS in Human Services field/focus of study preferred.
- 1-year experience in Care Management with primary care, Clinical Support System for Serious Mental Illness, (SMI) or similar populations.
- Must have a current Florida Driver's License with a clean driving record while meeting company insurance requirements.
