This person will be an advocate for clients and interact with clients in a manner that reflects cultural sensitivity. This position provides highly individualized services to clients that provides expertise and consults with other Team members to promote person-first culture, recovery, empowerment, self-determination and decision-making skills while utilizing the wraparound process. They will assure clients rights are protected and report any abuse and neglect.

The Unit Clerk is responsible for performing secretarial/clerical duties while providing top notch customer service skills. They function as the focal communication source in the nursing station. Provides indirect care in the department under the direction of the Unit Supervisor. Ensures Baker Act petitions and medication prior authorizations are completed as required. Performs detailed and accurate audits alongside the Unit Supervisor and compiles information. Maintains work area in neat and orderly manner, being mindful of infection control practices and safety initiatives established by the facility. Follows established policies and procedures relating to client confidentiality. Maintains office and hygiene supplies, completes work orders, and puts away department supplies keeping the department work area clean and organized as directed by the supervisor. Assists the Vice President of Acute Care and the Unit Supervisor, by completing other duties as assigned.

This position is responsible for providing great client care as the first contact for persons seeking inpatient Crisis Stabilization and Detox Services and assists throughout the admission process. Provision of direct client care and interaction under the supervision of the Director of Emergency Screening with significant training oversight by the Director of Access Center. Position requires heavy customer contact, teamwork skills, diligence in documentation, as well as independent critical thinking to manage sometimes multiple customer needs at the same time. The SSVF Housing Navigator will be a member of a multidisciplinary team in the Veteran Services program. This position will provide support to program participants to locate and secure permanent housing, this includes providing home visits and various field-based visits through enhanced housing services. The Housing Navigator will work closely with the HUD-VASH team to support veterans in their housing search and placement process. They will help to ensure that veteran that veterans are connected to the services which will assist in successful housing placement. The Housing Navigator supports the Supportive Services for Veteran Families Program’s efforts through landlord engagement, retention, and collaboration with other housing agencies. In this capacity, the Housing Navigator will provide landlord outreach and engagement services to develop a full range of permanent housing opportunities for homeless veterans. The Housing Navigator is the liaison for all landlords and property management personnel and the SSVF program and its clients. This position acts as a housing advocate for Veteran households by negotiating housing options on behalf of Veteran households in enrolled in the SSVF program. The position will support Veteran households in complying with housing rules through various tenant rights education and life skill trainings. Housing Navigator provide targeted customer service and mediation support to participating properties to promote long term tenancy and partnership and will work to find creative solutions to prevent tenant evictions and work to support positive tenant and Landlord resolutions.

The FACT Lite Manager oversees a multidisciplinary team to promote behavioral health, social support services, and medical care for patients/members, utilizing medical and health related professionals from numerous community organizations, as well as Meridian staff. The work is performed with a high degree of independence. The FACT Lite Manager is to train and guide staff to perform assessments and screenings, including identifying gaps in care, then co-develop long and short-term goals with patient/member for those dealing with severe and persistent mental illness, directly oversee staff’s performance in co-development of crisis intervention and resiliency plans (WRAP plans, relapse prevention plans) with patients/members. Provide oversight and care direction of Community Team Based wraparound models. The FACT Lite Manager is accountable for Multidisciplinary Team performance to coordinate and link client to receive the necessary services related to members’ comprehensive medical, behavioral, and social healthcare coordination needs. FACT Lite Manager and team perform services telephonically, virtually, in the community and or at patient/members residence with goal to improve clinical healthcare outcomes, improved consumer experience with healthcare systems and enhancements in facilitating client’s recovery and reductions in total healthcare costs.

The Family Intervention Specialist (FIS) functions as substance abuse treatment system experts and representatives to child welfare. FIS provide adult substance abuse outreach, screening, intervention, and case management. FIS do not function as the primary treatment counselor. Their role is to serve as a consultant to, and coordinator with, child welfare and a motivator/supporter for families. FIS may be co-located with protective investigation workers and shall maintain close contact with protective investigators and other child welfare workers. FIS maintain caseloads of up to 35 families. This position functions as a consultant and coordinator with protective service providers and the primary therapist. The Family Intervention Specialist provides resources, education, and motivational counseling to the client to promote family safety, prevent consequences of substance abuse on families under protective supervision or at risk of being under protective supervision by reducing drug and alcohol abuse, support parents/caregivers in completing case plans for permanency and reunification and recovery. This position often requires community-based services and a flexible schedule that will allow for work outside of normal business hours.

This is an independent professional position performing assessments, service brokerage, and advocacy services for adults with opioid use disorders who are in need of Medication Assisted Treatment (MAT) services. The Care Coordinator coordinates the care for consumers and works with other professionals from numerous community organizations, as well as with Meridian staff. This is a professional clinical position that reports to the Forensic Director to provide clinical function as a member of a Co-Responder team consisting of a behavioral health clinician and a sworn officer of Alachua County Sheriff’s Department. Majority of time will be spent with Law enforcement as ride along to intervene in community situations or crisis where Mental Health Issues are predominant issue. Assist the client to help prevent arrest or involuntary mental health inpatient stay. This position will include focus on individuals identified as high utilizers of crisis stabilization units, emergency rooms, and the Alachua County Jail. This person must be able to work independently and have the ability to work well with other agencies, such as the courts, jails, and community partners.

Customer service, computer, and crisis intervention skills are essential requirements of this position.

This position provide vocational services with a supported employment focus. The Vocational Specialist assists client with employment and career options to evaluate skills, work history and personality, matching a client’s education, experience, training and interests to available jobs. Services provided include but are not limited to Outreach Services, identifying potential participants, Screenings to determine program eligibility, Assessments to identify needs, Referrals-Service Linkage and Provision of Supportive Services to Assist veterans and their families obtain and or maintain social services, housing, emergency shelter, basic living items (food, clothing, furniture, personal hygiene, etc), employment, other income support, food stamps, job training, medical care. Also include, but limited to other supportive services such as: housing searches, housing inspections, transportation and other supportive services. This position is unique as staff will conduct outreach-engagement, case management and provision of supportive services in assigned services areas which could include the following service areas: Alachua, Bradford, Columbia, Dixie, Gilchrist, Hamilton, Lafayette, Levy, Putnam Suwannee, and Union, and must work as a liaison within the community and continually support the mission and standards of SSVF program and mission of Meridian. This is an independent professional position performing comprehensive Needs Assessments, service brokerage, and advocacy services for eligible Veterans and their family members in the Meridian service area. The Supportive Service Specialist works with other professionals from numerous community organizations, as well as with Center staff. The work is performed with a high degree of independence; however, work reviewed through conferences, supervision and required reports. This position also involves managing a caseload.

This is a highly responsible position in the Center’s Executive Management Team. The Senior Vice-President works with the Executive Management Team to ensure that care is safe, effective, person centered, timely, efficient, and equitable. S/he is expected to interact with stakeholders, internal and external, and is frequently required to represent Meridian’s executive management team to these stakeholders. In the absence of the President/CEO, and Executive Vice-President, the Senior Vice-President may be designated as the administrator in charge of the agency on a temporary basis. The Senior Vice President is responsible for managing a critical Center function: Operations, Finance, Human Resources, or Specialized Programs/New Business. The work requires considerable management, financial, and business expertise. The work requires the use of independent judgment and initiative in carrying out duties and responsibilities; while under the supervision of the President, work is primarily done independently.

A Therapist for the Community Action Team (CAT) provides clinical evaluation, treatment, and consultation services. It is a self-contained team that provides comprehensive, intensive, community-based treatment to families with children and youth at risk of out of home placement due to mental health disorders. This advanced professional position requires the ability to build professional empathetic relationships with clients, while providing efficient and appropriate direct clinical services. A CAT Therapist is expected to be an advocate for clients and interact with clients in a manner that reflects cultural sensitivity. Their purpose is to not only provide clinical treatment, but also help assure the clients rights are protected, report any abuse and/or neglect as mandated by law.

This is an advanced professional clinical position providing psychiatric services to Meridian's target populations. This position requires that the psychiatrist work with patients and within a multi-disciplinary staff to provide patient care and oversee services provided by lesser credentialed clinical providers. Work is performed with a high degree of independence and reviewed through observation, peer review, conferences and reports.
