The Court Diversion Coordinator is accountable for ensuring that the persons served receive all the necessary services that address the plan of care goals. The plan of care addresses comprehensive medical, behavioral, and social healthcare coordination needs. Health Home Coordinator performs services telephonically, virtually, courtroom, jail, in office, in community and at persons served residence.

The Vocational Specialist assists client with employment and career options to evaluate skills, work history and personality, matching a client’s education, experience, training and interests to available jobs. necessary information and linkage of information to access community resources. The Crisis Counselor is
a point of contact for information and materials to assist citizens to the right of entry to medical,
behavioral, and social healthcare coordination needs. Crisis Counselor performs services in all areas of the
community (faith based, businesses, events, schools, fairs). The Crisis Counselor is required to be
proactive during these public events including initiating conversation with citizens who may be suffering
wary of whom to trust and rather phobic, maybe even still, dismayed with the calm assurance to respond
to questions (closing assessment loop with citizen(s) if information is not immediately available and bring
them back to a place where follow up is most likely probable for their best outcome. The knowledge of
cultural group in the communities and ability to mingle in the various public forums to enhance Crisis
Counselor ability to gain trust and make progress with the amount recovery that is possible and how the
information disseminated ultimately will affect the communities served.

The Unit Clerk is responsible for performing secretarial/clerical duties while providing top notch customer service skills. They function as the focal communication source in the nursing station. Provides indirect care in the department under the direction of the Unit Supervisor. Ensures Baker Act petitions and medication prior authorizations are completed as required. Performs detailed and accurate audits alongside the Unit Supervisor and compiles information. Maintains work area in neat and orderly manner, being mindful of infection control practices and safety initiatives established by the facility. Follows established policies and procedures relating to client confidentiality. Maintains office and hygiene supplies, completes work orders, and puts away department supplies keeping the department work area clean and organized as directed by the supervisor. Assists the Vice President of Acute Care and the Unit Supervisor, by completing other duties as assigned.

This position is responsible for providing great client care as the first contact for persons seeking inpatient Crisis Stabilization and Detox Services and assists throughout the admission process. Provision of direct client care and interaction under the supervision of the Director of Emergency Screening with significant training oversight by the Director of Access Center. Position requires heavy customer contact, teamwork skills, diligence in documentation, as well as independent critical thinking to manage sometimes multiple customer needs at the same time. This position has potential for exposure to blood and body fluids.

The SSVF Housing Navigator will be a member of a multidisciplinary team in the Veteran Services program. This position will provide support to program participants to locate and secure permanent housing, this includes providing home visits and various field-based visits through enhanced housing services. The Housing Navigator will work closely with the HUD-VASH team to support veterans in their housing search and placement process. They will help to ensure that veteran that veterans are connected to the services which will assist in successful housing placement. The Housing Navigator supports the Supportive Services for Veteran Families Program’s efforts through landlord engagement, retention, and collaboration with other housing agencies. In this capacity, the Housing Navigator will provide landlord outreach and engagement services to develop a full range of permanent housing opportunities for homeless veterans. The Housing Navigator is the liaison for all landlords and property management personnel and the SSVF program and its clients. This position acts as a housing advocate for Veteran households by negotiating housing options on behalf of Veteran households in enrolled in the SSVF program. The position will support Veteran households in complying with housing rules through various tenant rights education and life skill trainings. Housing Navigator provide targeted customer service and mediation support to participating properties to promote long term tenancy and partnership and will work to find creative solutions to prevent tenant evictions and work to support positive tenant and Landlord resolutions.

The Family Intervention Specialist (FIS) functions as substance abuse treatment system experts and representatives to child welfare. FIS provide adult substance abuse outreach, screening, intervention, and case management. FIS do not function as the primary treatment counselor. Their role is to serve as a consultant to, and coordinator with, child welfare and a motivator/supporter for families. FIS may be co-located with protective investigation workers and shall maintain close contact with protective investigators and other child welfare workers. FIS maintain caseloads of up to 35 families. This position functions as a consultant and coordinator with protective service providers and the primary therapist. The Family Intervention Specialist provides resources, education, and motivational counseling to the client to promote family safety, prevent consequences of substance abuse on families under protective supervision or at risk of being under protective supervision by reducing drug and alcohol abuse, support parents/caregivers in completing case plans for permanency and reunification and recovery. This position often requires community-based services and a flexible schedule that will allow for work outside of normal business hours.

FACT Administrative Assistant serves as a liaison between participants and staff, including needs of office walk-ins and calls from participants and natural supports. A Therapist for the Community Action Team (CAT) provides clinical evaluation, treatment, and consultation services. It is a self-contained team that provides comprehensive, intensive, community-based treatment to families with children and youth at risk of out of home placement due to mental health disorders. This advanced professional position requires the ability to build professional empathetic relationships with clients, while providing efficient and appropriate direct clinical services. A CAT Therapist is expected to be an advocate for clients and interact with clients in a manner that reflects cultural sensitivity. Their purpose is to not only provide clinical treatment, but also help assure the clients rights are protected, report any abuse and/or neglect as mandated by law.

This is an advanced professional clinical position providing psychiatric services to Meridian's target populations. This position requires that the psychiatrist work with patients and within a multi-disciplinary staff to provide patient care and oversee services provided by lesser credentialed clinical providers. Work is performed with a high degree of independence and reviewed through observation, peer review, conferences and reports.
