Position Summary:

The Care Manager position is responsible for provision of family outreach, intervention, and case management. This position functions as a consultant and coordinator with protective service providers and the primary therapist. The services include advocacy, identifying resources and linkage.  This is an essential component to ensure rapid access to, and integration of, service components within Meridian and the community. The Care Manager works as part of an interdisciplinary team offering his/her expertise in identifying community resources for the family served to aid in the accomplishment of established recovery-oriented treatment and case plan objectives.

 Minimum Qualifications:

ยท  BA/BS from an accredited university with major course work in the areas of psychology, sociology, social work, health education or a related human services field. One-year experience in case management preferred. 

 ** Must be able to type at least 27 wpm on prescreen assessment and score at least 65% on general assessment. 

Drivers Requirement:

Must have a current Florida Driver's License with a clean driving record while meeting company insurance requirements.
In order to qualify as a Center Driver, the candidate must at least 21 years of age and have an active Florida Driver's license.  Motor Vehicle Record will be reviewed for eligibility.  No more than 3 moving violations and/or accidents within the past 3 years and no major violations during the past 5 years. Drivers over 65 years old must provide a current doctor's note stating they have no driving limitations. 

Essential Duties/Responsibilities:

Links the client and family with services and resources identified on treatment plan.

2.  Maintains contact with all staff/agencies providing services for the purpose of monitoring clients' progress and assessing appropriateness of services.

3.  Serves as an advocate for the client/family with a wide variety of community agencies and Center programs.  Promotes community program development to meet client needs.

4.  The Care Manager will provide resources for financial assistance to address crises or other needs.

5.  Prepares and maintains client records and insures quality of documentation of medical records. 

6.   Records every activity engaged on behalf of a client on a daily basis via Progress Notes.

7.   Researches, develops and maintains a comprehensive list of community resources that  includes addresses, phone numbers, and names of contacts for each resource.

8.  The case will be staffed weekly with treatment and family safety workers.

9.  Performs other duties as required to meet program and departmental goals and objectives.
