Florida Assertive Community Manager is to train and guide staff to perform assessments and screenings, including identifying gaps in care, then co-develop long and short-term goals with patient/member for those dealing with severe and persistent mental Illness, Manager oversee staff’s performance in co-development of crisis intervention and resiliency plans (WRAP plans, relapse prevention plans) with patients/members. Provide oversight and care direction of Community Based wraparound models. The Manager is accountable for Multidisciplinary performance to coordinate and link client to receive the necessary services related to members’ comprehensive medical, behavioral, and social healthcare coordination needs. Fact Manager and perform services telephonically, virtually, in the community and or at patient/members residence with goal to improve clinical healthcare outcomes, improved consumer experience with healthcare systems and enhancements in facilitating client’s recovery and reductions in total healthcare costs.

This position is responsible for the provision of direct client care and interaction under the supervision of a Registered Nurse (RN), Nurse Manager or physician. The LPN provides medication administration, wound care, patient and family education and historical data collection. Documents care provided in the clinical record.
